The OpportunityThis casual position will be responsible for supporting people aged 65 and over to remain living at home and engaging with them in social activities 1:1 and at times in group situations in our day centre. Hours are mainly Monday to Friday 9am to 4.30pm and at times evening and weekends. We are seeking male and female support workers and the following languages are desirable –Greek, Italian, Vietnamese, Mandarin, Cantonese, Hakka. ResponsibilitiesProvide in-home and/or centre based support to clients –this may involve providing transport, prompting with medication, supporting individual and group activities/social interactions.Provide support to clients in a way that promotes re-ablement (maintaining their independence)Assist with the completion of initial and annual care plan reviews and other client documentation as required.Report and record incidents.Follow the Food Safety Plan and complete relevant tasks and documentation to ensure compliance.Assist with continuous improvement by seeking feedback from participants and their carers. Skills & ExperienceCertificate III in Aged Care or equivalent (Disability, Lifestyle and Leisure etc)Experience supporting older adults in the community.Ability to understand the different approaches in communication and support to people who have a diagnosis of dementia, or other special needs.Demonstrated ability to work as a team member with a mature approach to resolving problems and conflict, and contribution to shared decision-makingAbility to communicate well – this means, clear verbal communication and good use of body language that can help the older person feel safe and secureAbility to undertake personal care tasks such as meal assist, medication prompting, assistance with hygiene and continence management, supervision to mobiliseAbility to access, read, implement and contribute to the evaluation of care plans as delegated and within the scope of Certificate 3 in Aged Care or equivalent competenciesGood working knowledge of Microsoft Office –particularly outlook, word and excel.A Victorian driver’s licence and a comprehensively insured vehicle. About Access Health & CommunityWe are a not for profit community health organisation operating in over twelve locations across Melbourne.  With caring at our centre, our mission is to build healthier lives together with our communities and deliver excellent health services for all.  AccessHC delivers medical, health and community services for all ages.  We employ over 250 employees across Eastern Melbourne. To ApplyPlease submit your application here or via the AccessHC careers page.
