The OpportunityAccessHC have commenced a new partnership with the Inner Eastern Melbourne Area of Department of Families, Fairness and Housing (DFFH) to develop capacity to become a provider of Multiple and Complex Needs Initiative (MACNI) Care Plan Coordination. The Pre-MACNI Care Coordinator will sit within the Alcohol and Other Drug Service at Access Health and Community. Care coordination will be provided to clients with complex needs. These clients would meet the criteria of over age 16 years, multiple diagnoses or disabilities, risky or unsafe behaviour and have intensive support needs.
