The OpportunityThis is an exciting opportunity to join a leading community health service as a Mental Health Nurse in the Stepped Care Mental Health program. This is a part-time, permanent position (3 days a week) with a possibility for an increase of work hours to 4-5 work days per week.The Mental Health Nurse will provide mental health nursing and clinical care coordination to consumers with moderate to complex needs in the Mental Health Stepped Care program. The Mental Health Nurse will work as a part of the multidisciplinary Mental Health team using the clinical staging model to assess and deliver mental health treatment to consumers. A key component of the role is the assessment and management of clinical risk for consumers living in the community with moderate to severe mental health needs. Key ResponsibilitiesUndertake clinical assessment of consumers which includes mental health, AOD, psychosocial, functional, and physical assessment domainsUse the clinical staging model in assessment and development of treatment plansWork collaboratively with consumers, family/carers and other health professionals to develop and implement Collaborative Care Plans and participate in shared careUndertake initial and ongoing risk assessments of clients and develop risk management plansProvide one-on-one clinical nursing services for clients with moderate to complex needs (including face-to-face, telehealth and outreach service delivery modalities)Monitor and support the consumer’s safe and appropriate use of medicationsCo-facilitate group-based programs where required Skills & ExperienceKey attributes of the successful applicant will include, but are not limited to:An empathic, innovative and professional therapeutic styleExcellent understanding of mental health symptoms, diagnoses and treatments across the lifespanCurrent and ongoing registration with Australian Health Practitioner Regulation Agency (AHPRA) as a mental health nurseDemonstrated experience in clinical risk assessment and management, including assessing mental state and clinical risk management across multiple domainsExperience/capability to work with clients presenting with co-occurring conditionsAbility to work creatively and safely with consumers from diverse backgrounds, including LGBTIQ, CALD and Aboriginal and Torres Strait IslandersAbility to work independently and as part of a multi-disciplinary team Culture & BenefitsPeople love working at Access Health & Community because we offer:Generous salary packaging opportunitiesOpportunity to purchase additional leaveCommitment to work-life balancePaid parental leaveMonthly ADO (for full time employees) About Access Health & CommunityWe are a not for profit community health organisation operating in twelve locations across Melbourne.
