We are seeking internal expressions of interest for a fixed term secondment to a new role at AccessHC - Senior Diabetes Connect Care Coordinator as part of the Diabetes Connect program. The Care Coordinator has a pivotal role providing client-centred assessment, referral coordination, and education to enhance self-management of pre-diabetes and type 2 diabetes. Following client centred assessment and intervention, our aim is to enhance clients' self-efficacy and reduce avoidable hospital admissions by facilitating timely access to clinical services and social support.As the Senior Diabetes Connect Care Coordinator, you'll collaborate with Service Connection, General Practice, Medical Specialists, Credentialled Diabetes Nurse Educators, Allied Health, Mental Health professionals, and Social Prescribing services internally and externally. Your role involves providing clinical guidance and supervision to future health coaches, and partnership with program managers to ensure successful program implementation, execution and evaluation.What you will be doingWork within professional scope of practice and service capability to provide timely, best practice and acceptable clinical care to clients for early intervention and self-management of pre diabetes and type 2 diabetes, including complications and co-morbiditiesOptimise client’s physical and mental health and social wellbeing through promotion of multi-disciplinary care, social prescribing and linkage to relevant community services and programs internal or external to AccessHCEngage with clients and carers to support self-management and goal directed care planningLiaise and correspond with relevant care providers such as referrers, general practitioners, diabetes specialists and allied health professionals; and escalate and advocate for services or intervention as relevantMaintain privacy, confidentiality and compliance with electronic health record documentation and administrative requirements of funding streams, and relevant Access Health & Community policies and procedures What you will bringMinimum 5 years’ clinical diabetes experience in a community health, hospital outpatient and/or private practice settingClinical knowledge and experience in delivering client centred care for chronic disease including self-management principlesDemonstrated ability to work collaboratively in a multi-disciplinary service and team environmentProven ability to relate to people from a diverse range of social, cultural and ethnic backgroundsProficiency in Microsoft Office and aptitude for learning and using relevant software such as TrakCare and Microsoft TeamsAttributesSkills in utilising Health Coaching and/or Motivational Interviewing techniquesUnderstanding of social prescription and importance of social connection for wellbeingUnderstanding of contemporary health landscape and funding models relevant to community health setting and chronic diseaseEffective time management and clinical care prioritisation skillsStrong interpersonal, written and verbal communication skillsDemonstrated behaviours consistent with AccessHC valuesExpress Your InterestPlease submit a cover letter and current resume to address the key selection criteria.
