The OpportunityWe are seeking internal expressions of interest for a fixed term secondment to a new role at AccessHC – Diabetes Connect Health Coach as part of the Diabetes Connect program. An overall aim of the Diabetes Connect is to enhance clients' self-management and reduce avoidable hospital admissions by facilitating timely access to health education, clinical service and social support.Under the guidance and delegation of a Diabetes Connect Care Coordinator and the client's health professional team, the Health Coach will work closely with clients to support access to health and social services, including referrals internally and externally, coach clients to implement agreed care plans and actions, and enhance client self-management to achieve their health and wellbeing goals. Maintaining an electronic health record of your interaction with clients and referrals, and regular liaison with the Care Coordinator to review client progress is required, along with statistical recording of your time.What you will be doingSupport clients to action their agreed care plans with regular health coachingGuide clients to build on their skills and confidence to enhance self-management of their health conditionsWork with Care Coordinators to coordinate referrals to health and community services, including social prescribingRegularly review current health status and progress toward goals with clients, and escalate any concernsCollaborate with clients and relevant health professionals as part of a multidisciplinary model of careLiaise with internal and external service and program providers relevant to client referrals and careMaintain privacy, confidentiality and compliance with electronic health record documentation and administrative requirements of funding streams, and relevant Access Health & Community policies and proceduresWhat you will bringMinimum 3 years’ experience working as an Allied Health Assistant or Enrolled Nurse in a client facing role in a health care settingDemonstrated ability to work collaboratively in a multi-disciplinary service and team environmentEffective time management and task prioritisation skillsProven ability to relate to people from a diverse range of social, cultural and ethnic backgroundsProficiency in Microsoft Office and aptitude for learning and using relevant software such as TrakCare and Microsoft TeamsAttributesWillingness to develop and use health coaching or motivational interviewing techniquesInterest to incorporate social prescription and understand the importance of social connection for wellbeingWillingness to learn about the contemporary health landscape and funding models relevant to community health setting and chronic diseaseStrong interpersonal, written and verbal communication skillsDemonstrated behaviours consistent with AccessHC values Express Your InterestPlease submit a cover letter and current resume to address the key selection criteria.
