Our qualified staff provide individualised care and support to assist each client (and their carers) to stay active, connected, involved and independent at home.We are seeking an individual with a background in aged care, to undertake initial and ongoing assessments. Develop care plans in partnership with clients and their carers requesting in-home and centre based respite. Assessment and planning includes consideration of risks with a focus on optimising health and well-being in accordance with the client’s need, goals and preferences and reablement framework. You will collaborate with support staff and the team leader to ensure that care plans accurately reflect client background, current interests, support needs and strategies to ensure client needs are met during respite. This role will also include the provision of direct support to clients during in-home or centre based respite, at high volume periods or when backfill required for staff leave. What you will be doing Visit clients in their homes to assess their care needs including risks, understand carer’s goals and agree on respite services to be provided including the respite fee, transport arrangements, days and times of respite and agreed in-home respite tasks.Create a care plan once the assessment is complete.
