The role is based in Doncaster East and involves arranging Allied Health appointments across Inner East and North East Melbourne. Utilise your excellent communication skills providing an initial point of phone contact for clients requiring Allied Health services for their improved quality of life. Intake provides support and information to the wider community regarding Access HC’s integrated services. We provide a timely and client centred service which is critical when processing referrals, linking our clients with services, booking appointments and providing clients with additional, relevant information where required. Responsibilities Liaise with Internal and External referrers Book Initial & Review appointments for clients Complete Initial needs identification assessments for new clients Answer Incoming telephone enquiries Accurately provide clients with verbal and written communication regarding services Register client information into TRAKCare Create episodes of care Support clients with linking in with alternative supports where required Liaising with Interpreter services Skills and Experience Experienced in operating cloud based telephone systems Ability to establish rapport and develop respectful, professional relationships with clients, staff and partner organisations. Have a high level of energy, enthusiasm and flexibility, along with a commitment to team work Demonstrated ability to work independently while managing a range of tasks Understanding of Community Health and Allied Health Services Strong communication and computer software skills, such as Microsoft Office.
