We are seeking a dedicated professional to join our team in a flexible, full-time Max term until June 2026 to deliver specialist care finder services to assist older adults with engagement barriers in understanding and accessing aged care and community support. Reporting to the Manager Community Access and Outreach, you will provide intensive support to seniors who struggle to navigate the system independently and help prevent vulnerable individuals from falling through the cracks by connecting them with essential services.
