The Opportunity
Make a Difference in Community Health
Access Health and Community (AccessHC) is seeking a skilled and compassionate Practice Nurse (Registered Nurse Grade 2) to join our Medical Services team.
This is an exciting opportunity to work in community-based primary care, supporting General Practitioners while delivering high‑quality, patient‑centred nursing care in a collaborative and values-led environment. Reporting to the Practice Manager, this is a Max Term Fulltime Parental Leave position working Monday – Friday (4th May 2026 – 24th December 2026) primarily based at our GP Clinics in Richmond & Hawthorn.
About the role
The Practice Nurse plays a critical role in supporting clinical service delivery across AccessHC Medical Sites. You will work closely with General Practitioners and other health professionals to ensure safe, efficient and coordinated care for clients. The role includes clinical care, care coordination, administration, quality improvement activities and participation in accreditation processes. The role requires the flexibility to work across the sites and cover leave when required.
This is an excellent opportunity for a motivated nurse who enjoys variety, autonomy and working within a multidisciplinary, values-driven organisation.
