About the role
An exciting opportunity to lead the implementation and expansion of the Care Navigators Program, embedding integrated care navigation services within GP practices across the Eastern Melbourne Primary Health Network (EMPHN). Lead a Care Navigators team, ensuring best-practice service delivery, operational excellence, and stakeholder collaboration. Scale the program from pilot to full implementation, reaching 60 GP practices by June 2026, while driving best-practice, patient-centered healthcare solutions. Build strong partnerships with General Practices, EMPHN, and the University of Melbourne to enhance patient outcomes. Join us in transforming primary healthcare through innovative, integrated care solutions.
