About the role
We are seeking a Community Health Nurse to deliver in collaboration with the Service Manager and Community Outreach Team, an Outreach program within the local government areas of Boroondara, and Manningham. Collaborating with Community Engagement workers. You will facilitate supportive connections and referral pathways to vulnerable people who are experiencing isolation, marginalised housing or homelessness, and health issues to help them build trusted connections with health services and social activities. This is a Permanent Part-time 0.6 EFT position reporting to the Manager, Community Access and Outreach.
