About the role

We are seeking a Community Outreach and Engagement Worker to join our Community Impact team in a permanent part‑time role (0.5–0.6 FTE) Tuesday, Wednesday & Thursday based in Hawthorn or Doncaster. What you will be doing

  • Engage with Community members otherwise hardly reached, by connecting them with community, social and health services
  • Build trusting relationships with community members and support access to health, social and community services
  • Negotiate referral pathways internally and externally to ensure people who meet eligibility criteria are identified and supported to access programs and services
  • Assess risk and needs, providing appropriate referrals, escalation and follow‑up as required
  • Promote wellbeing, health and community supports available across AccessHC and partner organisations
  • Collect, record and evaluate program data, maintaining accurate case notes, reports and documentation
  • Collaborate with team members in the provision of quality meaningful support

What you will bring – Key Selection Criteria

In addition to your passion and purpose to support vulnerable people within our community you will have. 

  • Minimum 1 year experience in community outreach programs particularly with hardly reached community members
  • Social Work or Community Development qualification
  • Demonstrated experience working with clients who experience mental health challenges in a community setting
  • Team Player with previous experience working collaboratively in a diverse and multifaceted team where shared decision making is required 
  • Demonstrated experience engaging with a range of stakeholders including GP and health services, AOD services, consumers and community groups

Desirable

  • Proficiency in spoken languages other than English especially Arabic, Farsi, Cantonese, Mandarin and Vietnamese desirable
  • Knowledge of NDIS and MAC systems highly desirable
